IF YOUR STUDENT HAS DRIVER'S ED OR NO PE CLASS ON MAY 9TH, they may go during a free period (lunch, study hall, etc.) or sign up here to come on Wednesday evening to be screened as a student model for our volunteer training (please note that you must ALSO register them for the actual screening at the YH4L link below):  https://signup.com/go/VgJo5Q

 

To sign your child up for this FREE cardiac screening,
please go to www.yh4l.org

1. Click on the drop-down Menu that says Registration & Events and choose Registration & Results. 

2. Click under Chicago Region - High Schools.

3. Read the YH4L information and then click on the “I Am Giving Permission for my child” Icon (located at the bottom of the page).

4. Complete all required registration information on the next page with an asterisk(*), and choose Oak Park River Forest High School.

Online registration will close at 12:00 pm the day before the screening.

Please DO NOT register your child at a different school.
